For after hour consultations please speak with the receptionist who will arrange a suitable time with the provider. We are closed on Victorian Public Holidays.
If your medical problem requires attention on the day please advise the receptionist who will ensure that you speak promptly with the nurse.

Appointments are required to see the service providers. We have same day appointments available and in the case of an urgent appointment required you will speak promptly to the nurse. If the appointment is going to take longer than a normal standard appointment, please advise the receptionist when booking. You can also make a booking enquiry using the form on our online booking facility.
If you are unable to attend your appointment please phone and speak with the receptionist as soon as possible before your appointment time. We can then assist you with rescheduling your appointment and we can also offer your appointment time to a another patient who is on the waiting list.
Youth Related Appointments
Active Health Portland have appointments available for youth related issues, and appointments are available on the day. Appointments will be made suitable to the patient where possible.
Fees and Billing
39.629Active Health Portland’s General Practitioners offer bulk billing for pensioners, health care card holders, students (up to age of 25), children and DVA Card Holders.
Allied Health Service Providers have varied fees which you can learn more about by visiting “Our People and Services” on this website and checking the Service Providers page.
For private patients you will be required to pay the full fee for your consultation and you will then be able to claim most of this fee back through Medicare. If you have your bank account information registered with Medicare we will be able to process your rebate from Medicare at the time of payment and it will then go back into your bank account within 48 hours. It is essential, however, that you ensure your account details are correctly registered with Medicare as the account you have nominated with them will be the account that your rebate will go into.
If you do not have your bank account registered with Medicare you can access the Medicare Bank Account Registration Form here. Once completed you will need to take this form to Medicare and they will set up the information they require so that your rebate from a variety of service providers including Active Health Portland can be directly credited to your nominated account.
All fees are charged at the doctor’s discretion. If you have any difficulty with payments or are in financial stress, please speak to the doctor, Allied Health service provider or practice manager for assistance.
If you miss an appointment or cancel giving less than 24 hours notice you may be charged a fee.

Health & Privacy Information
Your medical record is strictly confidential. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff.
Our practice uses a computerised medical record system to record your notes and personal information. These systems are all password protected and backed up daily. All information recorded in our clinic is strictly confidential and is not available to other surgeries without your written permission.
We are governed by the Privacy Act 2000. Our Privacy Officer is the General Manager who can be contacted on (03) 5522 1200.
Freedom of Information
You have the right to access your medical record under the Freedom of Information Act 1988. You may ask for copies or come and view your health record in person. Viewing of your medical record is done with the Doctors assistance who will be able to clarify anything you do not understand. Please speak to our Practice Manager if you would like an application form for ‘Access to Personal Health Information’. A fee may be charged for accessing and/or receiving copies of your medical record.
Health Assessments
The aim of these assessements is to prevent illness before it occurs and can be done at a variety of ages throughout a patients lifetime.
These assessments include:
- child health checks for 4 years
- 40-49 years health assessments
- 75+ years health assessments
- ATSI health assessments
- Intellectual Disability health assessments
- Home Medication Management Reviews

A personally controlled eHealth record is a secure online summary of your health information. You control what goes into it, and who is allowed to access it.
Your eHealth record allows you and your doctors, hospitals and other healthcare providers to view and share your health information to provide you with the best possible care.
We now have information available at reception about how your eHealth record can be combined with your visits to Active Health Portland.

Employment Medicals
These medicals can involve many parts including:
- basic musculoskeletal
- skin checks
- hearing
- vision
- respiratory testing
- pathology (including urine drug & alcohol screening)
- ECG’s and
Bookings will include seeing the Practice Nurse and Doctor.
All testing is conducted on the same day and results emailed or faxed to you directly. This eliminates any waiting period.
Employment medicals can be completed using:
- your company’s form
- our own specially developed pre – employment medicals
- or they can be tailored to suit your business needs
