Health Independance Program


Our mission is to create an integrated, effective and sustainable chronic disease and complex needs programs, which provides the right care in the right setting, to reduce avoidable hospital admissions and contribute to better health outcomes for the Glenelg shire community.

The health Independence guidelines have been developed by the Department of Human Services to provide direction and facilitate alignment for health independence programs that include:
