Community Transport

Portland Community Transport Service is a transport service provided by Portland District Health to eligible clients.

The service is provided with assistance of volunteers and is available for eligible users for medical appointments and access to community services. The service can also be used for visits to friends and relatives in hospital, or care and for one-off-social visits.

All clients who wish to use the service must meet eligibility criteria to be able to access transport. This may include:

  • Frail aged persons and/or those with physical, sensory or intellectual disabilities who have a functional disability;
  • Lives at home;
  • Cannot use conventional public transport;
  • No other transport options open to them including Red Cross Transport Service and Public Transport;
  • Are financially disadvantaged.

People over the age of 65 (or 50 for aboriginal or Torres Strait islander people) will need to go through My Aged Care. Clinicians can do this on behalf of the person with consent, to the access the web referral visit

Any member of the public can contact the service without referral or a service provider can contact the service on behalf of the client.

Please give as much notice as possible and the co-ordinator will contact you by telephone.
